  • Continued one-on-one support focused on helping you navigate ongoing challenges, life changes, and next steps. Sessions may include resource coordination, provider communication support, organization assistance, advocacy guidance, application follow-up, problem solving, and personalized planning based on your current needs.

  • This may include assistance with applications, renewals, referrals, provider coordination, paperwork guidance, and connecting clients with community based supports and programs such as SNAP, Medicaid, WIC, Section 8, income-restricted housing, and other local/state services. Support is personalized to each client’s unique situation with a compassionate, judgement free approach focused on reducing overwhelm and helping clients feel more supported in the process.

  • This service focuses on helping clients better understand their options, communicate effectively with providers or agencies, organize next steps, and feel more confident navigating systems and decisions. Whether you need help understanding resources, preparing for conversations, managing paperwork, or simply having someone in your corner while navigating challenges, support is provided with compassion, patience, and a client-centered approach.

  • Practical administrative support designed to help reduce overwhelm and keep important tasks, paperwork, and information organized. Services may include form completion assistance, application tracking, document organization, appointment scheduling support, phone call assistance, email or correspondence help, resource documentation, printing/faxing/scanning coordination, and other administrative tasks related to care, services, or daily life management. Support is tailored to each client’s individual needs with a compassionate and judgement free approach.
